Overview Central and Eastern Sydney PHN (CESPHN) is seeking Expressions of Interest from eligible small and solo (1-5 GP FTE) general practices located in Sydney's Inner West to participate in the Diabetes Care Clinic Program, supporting patients with prediabetes and type 2 diabetes. Scope The scope includes: Participation in a multidisciplinary model of care for patients with prediabetes and type 2 diabetes Connecting patients with diabetes-related allied health services delivered by Diabetes Australia, either onsite at the practice or virtually Providing these allied health services to patients at no cost The program will run through to June 2027. Funding General practices selected to participate will receive $5,000 (excluding GST) in funding support.
