Overview Northern Territory PHN (NT PHN) is seeking applications for the General Practice in Aged Care Incentive (GPACI) Thin Markets Funding Grant through a competitive Request for Proposal (RFP) process. Purpose The GPACI Thin Markets Funding Grant is a time-limited pilot with two streams, GP and allied health, designed to improve access to high-quality and sustainable primary care services for permanent residents of eligible residential aged care homes (RACHs) in identified thin-market regions of the Northern Territory.
