Overview Country to Coast Queensland (CCQ), the Primary Health Network (PHN) for Central Queensland, Wide Bay, and the Sunshine Coast region, is seeking to engage a provider to deliver the Balance for Life: Bundaberg Healthy Ageing and Falls Prevention Program. About There are 31 Primary Health Networks throughout Australia, which are independent organisations funded by the Commonwealth Department of Health, Disability and Ageing. PHNs work to streamline health services, particularly for those at risk of poor health outcomes, and to better coordinate care so people receive the right care, in the right place, at the right time. CCQ is the PHN for Central Queensland, Wide Bay, and the Sunshine Coast region. Objectives Improve participant strength, balance, confidence, and falls related knowledge Reduce falls risk and support independence and general healthy ageing Encourage greater social connectedness Scope The scope includes: Delivery of a high-quality, evidence-based falls prevention program for older people Provision and promotion of clear, comprehensive and responsive referral pathways for Bundaberg general practices, hospital and health services, Queensland Ambulance Service, community organisations and aged care providers, including mechanisms for self-referral Collection and reporting of agreed performance and outcomes data Working collaboratively with CCQ and regional stakeholders Instructions to Tenderers Interested providers should download the RFP documents and submit their application.
