Overview Lake Maintenance (Western Australia) Pty Ltd is requesting subcontractor quotes for an upcoming project at 137 Graeme Street, Kalgoorlie (GF-13), to be included in its QR4 tender submission to the Department of Housing and Works (DoHW). Scope Works and technical specifications as required by the Department of Housing and Works (DoHW), as detailed in the attached documents, at 137 Graeme Street, Kalgoorlie (GF-13) Information Requested Submissions must include the following to be considered for the QR4: Completed Subcontractor Evaluation Questionnaire Quote clearly showing price, excluding GST Quote valid for 30 days Quote including lead time (in weeks) to mobilise and time (in weeks) to complete all works as per the Scope of Works, including any holiday closing period Price breakdown using the supplied template (MDT RFQ price breakdown) Safety Management Plan Indication of whether a progress payment arrangement is to be considered Contractor Requirements Public Liability Insurance (minimum AUD 20 million) Workers Compensation Insurance (minimum AUD 20 million) All subcontractors, their employees, and contractors must complete onboarding inductions prior to commencing work on site Subcontractors must follow the appropriate protocols when entering and leaving remote communities Instructions to Tenderers Quote submissions must reflect the requirements set by the Department of Housing and Works, as detailed in the attached documents. Any submission deemed incomplete will be rejected. Works will only be deemed completed and paid after all scope requirements are submitted, including before and after photos, waterproofing photos, and reports. If your company is unable to provide a quote submission, please reply to the request email briefly explaining the reason, selecting from: A) Pricing, B) Current Capacity and Workload, C) Nature of Works, or D) Resources. Progress payment terms must be recommended by the Head Contractor at the time of submitting quotes to the Principal. These terms are negotiable but must be agreed with the Principal prior to any works being awarded. Submission of quotes should be made no later than 5:00 PM Wednesday 04/11/26.
