Overview Transport for NSW (TfNSW) has issued a Request for Proposal (RFP) for the TfNSW Fitness to Drive - Online Medical Forms (FTD-OMF) project, RFP WS5682184941. Purpose If a driver or rider has a medical condition that affects their ability to drive or ride safely, or they reach a certain age, they are required to have a health professional assess their Fitness to Drive on a regular basis. If there is a change in their health circumstances, TfNSW needs to be notified. The Online Medical Form project provides a solution for health professionals to complete and submit online forms related to supporting Fitness to Drive decisions for Transport, with real time interaction between the health professional's medical practitioner systems and TfNSW. Scope TfNSW expects the vendor to provide the following key components: Form Specification and Design: working with TfNSW stakeholders to define and refine the assessment forms, noting a need to drive continuous improvement and increased automated processing rates Form Build: working from the form specifications to build the assessment smart forms Form Hosting: hosting assessment forms and facilitating access to all vetted Health Professionals, via commercial practice management software and the vendor's own website/application Instructions to Tenderers In order to gain access to the RFP sourcing event in ARIBA, tenderers are required to register their interest by sending an email to TSS.Bidder@transport.nsw.gov.au and include the following: RFP No and Title: TfNSW Fitness to Drive – Online Medical Forms (FTD-OMF) RFP RFP ID: WS5682184941 Organisation Name ACN/ABN Contact Name Organisation/Contact postal address Contact Email Address (preferably an email address containing the contact person's name) Contact Phone Number To gain access to the sourcing event in ARIBA over the internet once access is provided, tenderers must use a PC with one of the following web browsers: Google Chrome 54+ (64-bit) Microsoft Internet Explorer 11 (32-bit) Mozilla Firefox 24 or 27 (ESR) Safari 9+ (64-bit) Microsoft Edge (32-bit) Tenderers are required to submit a correctly signed Confidentiality Deed Poll (CDP) for TfNSW review and acceptance prior to gaining access to the RFP documentation in ARIBA.
