Overview Northern Territory PHN is seeking suitably qualified providers to deliver the Multidisciplinary Team (MDT) Support Services Program in Alice Springs, providing group-based support services that complement MDT clinical care for people living with diabetes, cardiac disease and/or pulmonary disease. Purpose The objective of this Request for Tender is to identify suitably qualified providers to deliver the MDT Support Services Program in Alice Springs, complementing MDT clinical care and improving health and wellbeing outcomes for people living with diabetes, cardiac disease and/or pulmonary disease. Scope The scope includes: Delivery of semi-structured group activities to eligible participants that support chronic disease self-management Activities to improve functional capacity and encourage healthy lifestyle behaviours Activities to strengthen peer support and reduce the risk of preventable hospitalisations Delivery of services in collaboration with the MDT System Navigator and other MDT providers to ensure coordinated, integrated care Tender Briefing An information session will be held for this opportunity.
