Select request for tender: Brimbank general practices for the Health Care Neighbourhood Project

Overview North Western Melbourne Primary Health Network (NWMPHN) is seeking to commission two additional general practices to deliver the Health Care Neighbourhood Project in the Brimbank local government area. Scope The scope includes: Enhancing multidisciplinary care within the practice Working with a three-person neighbourhood care team in the practice at least one day each week Funding Each successful practice will receive $108,225 (ex. GST) until 30 June 2029 Instructions to Tenderers A select request for tender (RFT) is now open through NWMPHN's tenders portal, Tenderlink.
