About this tender
Overview Adelaide Primary Health Network (Adelaide PHN) is inviting Residential Aged Care Home (RACH) organisations to take part in a free Change Management Training Program delivered onsite at their location. Funding is available for 15 Residential Aged Care Home Organisations. Scope Successful organisations will receive: Two onsite change management workshops delivered by Thrive Aged Care Consultants Practical tools and strategies to lead and sustain change Support to implement a real service improvement initiative Assistance to increase telehealth capability and adoption within the service The program is delivered by Thrive Aged Care Consultants and provides practical, hands-on support to help teams embed digital health into everyday practice. Eligibility Open to Residential Aged Care Home Organisations Funding is available for 15 organisations How to Apply The Expression of Interest (EOI) period has been extended and EOIs are now site-based, giving individual RACHs flexibility to nominate their own team.
