Overview Lake Maintenance (Western Australia) Pty Ltd is requesting quotes from subcontractors for refurbishment works at 24 Weld Street, Northam, in connection with the QR4 tender process. Quotes must be sufficient to cover the required works and technical specifications as per the Department of Housing and Works (DoHW) requirements. Scope Refurbishment works at 24 Weld Street, Northam, in accordance with the Scope of Works (SoW) and the supplied technical specifications required by the Department of Housing and Works (DoHW) Information Requested Submissions must include the following to be considered for the QR4: Completed Subcontractor Evaluation Questionnaire Quote clearly showing price, ex GST Quote valid for 30 days Quote including lead time (in weeks) to mobilise and time (in weeks) to complete all works as per the SoW, including any holiday closing period over the Christmas period Price breakdown using the supplied template (MDT RFQ price breakdown) Safety Management Plan Evidence of Public Liability Insurance (minimum AUD 20 million) Evidence of Workers Compensation Insurance (minimum AUD 20 million) Indication of whether a progress payment arrangement is to be considered (Yes/No) Contractor Requirements Public Liability Insurance of a minimum AUD 20 million Workers Compensation Insurance of a minimum AUD 20 million All subcontractors, their employees, and contractors must complete onboarding inductions prior to commencing work on site Subcontractors must follow the appropriate protocols when entering and leaving remote communities Compliance with the requirements set by the Department of Housing and Works, as reflected in the quote and supporting documents Instructions to Tenderers Submission of quotes should be made no later than 5:00 PM Thursday, 24/09/2026. If a company is not able to provide a quote submission, a reply must be sent explaining the reason, selected from the following options: Pricing, Current Capacity and Workload, Nature of Works, or Resources. Progress payment terms must be recommended by the Head Contractor at the time of submitting quotes to the Principal. These terms are negotiable but must be agreed with the Principal prior to any works being awarded. Any submission deemed incomplete will be rejected. Works will only be deemed completed and paid after all scope requirements are submitted, including before and after photos, waterproofing photos, and reports.
