About this tender
Overview Healthy North Coast is seeking Expressions of Interest from suitably qualified organisations to deliver the Clarence Valley Health Access Initiative, Navigation, Community Connection and GP Liaison Service. The service will support people across the Clarence Valley to access the right health, community and social services at the right time. Scope The scope includes: Delivery of community navigation and connection support across the Clarence Valley Working directly with general practices to identify patients who may benefit from additional support Assisting with referral processes, including warm referrals and follow-up where required Connecting patients with appropriate local services Contractor Requirements Strong local knowledge of the Clarence Valley Established community relationships Experience in navigation, referral support, care coordination, community development, integrated care or similar service models Partnership, consortium or subcontracting arrangements may be proposed where this strengthens local reach and service delivery Funding Total funding available is $450,000 excluding GST.
