About this tender

Overview South Western Sydney Local Health District (SWSLHD), via HealthShare NSW, seeks to establish a panel of providers to deliver approved Transitional Aged Care Program (TACP) services across the SWSLHD. Objectives The objectives of the Transitional Aged Care Program (TACP) are to: Support older people to maintain functional independence Support achievement of person-centred rehabilitation goals Facilitate safe discharge from hospital to home Reduce avoidable hospital readmissions Delay or prevent premature admissions to residential aged care Support transition to longer-term community support where required Scope Further detail is set out in the document named "SWSLHD_5504 - Part B - Specifications". The successful provider(s) must be capable of delivering the following: Home care services, including personal care, showering assistance, assistance with activities of daily living, domestic assistance, household cleaning, light errands, transport to appointments, community access support, meal preparation, welfare and wellbeing checks, and support aligned with approved TACP care plans Community nursing services Podiatry services The following services are excluded from this procurement: Acute hospital services Residential aged care services Long-term aged care services Equipment supply and rehabilitation aids Tender Briefing A briefing session will be held for tenderers to answer queries regarding this RFT and the RFT process.
