About this tender
Murray PHN has opened a small grants capacity building opportunity to eligible community pharmacies and registered pharmacists to improve access to aged care onsite pharmacists in Residential Aged Care Homes (RACH) within the Murray PHN catchment. There are two parts to the grant: Part A - ACOP Readiness - to build capacity to enable readiness for delivery of ACOP services to Residential Aged Care Homes (RACH) Part B: ACOP Participation - to support commencement or expansion of delivery of aged care onsite pharmacist services to RACH located within the Murray PHN catchment. Eligible pharmacies and pharmacists must be located within one of 22 local government areas (or in bordering communities) within the Murray PHN catchment and have a commitment to providing Aged Care Onsite Pharmacist services under the Aged Care Onsite Pharmacist Measure. As the number of available grants is limited, applications will be considered and awarded as received (based on the date and time of receipt in the etender portal). Applications may close earlier if all grants are awarded before the closing date.
